Home Healthcare Operational Workflow Case Study

This project focused on improving how home healthcare agencies manage field operations and clinical documentation. Teams relied on spreadsheets, WhatsApp, and paper, leading to fragmented data, poor visibility, and high manual effort. Through field research and service design, I mapped current workflows, identified root causes, and co-designed future processes with operations and clinical teams. The outcome was an integrated service model with structured care plans, clearer ownership, real-time visibility, and system requirements to support scalable, reliable care delivery.
